Advance directives

If you're unable to make your wishes known, your loved ones and health care providers can refer to an advance directive when they make decisions about your care.

An advance directive is a legal document provided by the state that outlines your wishes for medical treatment ahead of time. The document’s purposes are to:

  • Specify the treatments you want or don’t want during end-of-life care
  • Name a health care agent you trust to make medical decisions for you if you can no longer speak for yourself
