How to access your medical records

As a patient, you have the right to access your medical records. We offer that access through our patient portal or by requesting a copy in paper or electronic format. You can have your records sent to yourself or a third party, such as another health care provider or employer.

Viewing your medical records on the patient portal

The MyAtriumHealth patient portal gives you access to your medical records online anytime, anywhere. You can also view test results, request prescription refills, pay a bill or message your provider.

Request medical records from Atrium Health Floyd

To request medical records for services received at Atrium Health Floyd, call 706-509-6185 or email medicalrequestfloyd@atriumhealth.org.

Request medical records from Harbin Clinic

To request medical records from Harbin Clinic, download our authorization for the release of medical records. Send the completed form to Health Information Management by fax to 706-291-9782 or mail to 221 Technology Pkwy., Rome, GA, 30165.

Note: To have records from another facility sent to us, download and complete our authorization to obtain outside medical records.

Is there a cost for medical records?

If you have your medical records sent directly to a doctor or other health care provider, there is no fee. For other types of requests, there may be a reasonable fee.
