Nominate a nurse

Every day, nurses make a profound difference in the lives of patients and families – often in ways that go far beyond clinical care. They help people feel safe, cared for, and understood.

If a nurse made a difference for you or someone you love, writing a DAISY nomination is a great way to say thank you. Your story can show others how that nurse was kind, helpful, and supportive when it really mattered. By sharing what happened, you help honor a nurse who truly made your day better. Use these prompts to write your story:

  • P – Person centered in the care they provide. How they considered your individual needs as a patient/guest in the situation.
  • E – Empathy in the situation. How did the nurse show care and understanding for the person or situation?
  • T – Trust and Teamwork of families, patient and team. How the nurse was there for you and made you feel safe.
  • A – Admirable qualities shown. Are there special qualities this nurse has that other people would look up to?
  • L – Listened, heard and understood. In what way did they listen and help you?
  • S – Standout. What made this nurse special?
